The OnTrack Hamilton Master's ClinicianDuties:
- Perform quality work within deadlines with or without direct
supervision.
- Interact professionally with other employees, customer and
vendors.
- Work effectively as a team contributor on all assignments.
- Work independently while understanding the necessity for
communicating and coordinating work efforts with other employees
and organizations.
- Conducts individual, family, and group counseling
- Is responsible for maintaining a designated amount of direct
service time with clients and a designated amount of contacts for
the assigned caseload.
- Participates in the initial individual program planning process
with all service providers (i.e., Outreach/Education specialist,
SEES, and medical staff) and provides ongoing information regarding
client progress to the service providers.
- Completes an individual treatment plan with each client within
30 days of enrollment.
- Writes progress notes after each service contact with a client
in accordance with the Center's procedure on Time Frames for
Clinical Records and Related Documentation.
- Maintains documentation of a six-month service update in the
clinical record.
- Participates in the development of a crisis/safety plan for
priority clients.
- Participates in regularly scheduled Peer Review.
- Follows all guidelines outlined in the HRMC Policies and
Procedures Manual including Corporate Compliance Policies.
- Is expected to have regular and predictable attendance and the
ability to work cooperatively with others.
- If licensed, signing the non-licensed clinicians' treatment
plans and being available for clinical consult
- Coordinates and schedules ongoing outreach/referral meetings
with community providers as part of the OnTrack model.JOB
PURPOSE/SUMMARYSummary of role of team:
- Clinician works in a team format to meet the individual needs
of clients in the program, while providing clinical recommendations
for treatment.
- Functions as a member of a treatment team to plan, implement,
and evaluate successful interventions for individuals and families.
Summary of position:
- Serves as the primary clinician that provides individual,
group, and family counseling as part of the First Episode
Psychosis/On Track TN model.
- Clinician provides weekly individual, group, or family
psycho-therapy to adolescents and young adults ages 15-30 with a
mental health diagnosis that involves symptoms evident of
psychosis.
- Clinician will help clients clarify goals, cope with stressful
situations, interact more effectively with other people, and
generally to overcome barriers to client's recovery.
- Clinician is to provide psycho-social assessments for clients
and provides diagnosis and appropriate referrals for clients
assessed, as needed.
- Clinician provides crisis intervention and emergency services
as needed. The primary clinician will rotate in a 24 hour/7 day a
week on-call schedule.
- Treatment modalities used by Master's level clinicians
providing therapy services include Dialectical Behavior Therapy
(DBT), Cognitive Behavior Therapy (CBT), CBT for psychosis (CBTp),
Trauma-Focused Cognitive Behavior Therapy (TF-CBT), Attachment,
Self-Regulation and Competency (ARC).
- Clinician embraces the key values of clinical services:
empowerment, normalization, rehabilitation, and continuity of
care.
- Provides administrative duties for the OnTrack TN program
including supervisory responsibilities, training of new staff
members and continued staff development, monthly reporting in house
and to the state of Tennessee (TDMHSA), and the promotion of the
program with outside agencies.
- Conducts administrative and clinical supervision to a
coordinated specialty care team.JOB DUTIES/RESPONSIBILITIESQuality
Care
- Provides individual/family psycho-therapy for youth and young
adults ages 15-30.
- Clinician is present and ready to begin session at the
appointment time, every session.
- Clinician calls to reschedule any sessions that he/she will be
unable to attend or that the consumer missed or canceled 100% of
the time.
- Clinician communicates accurate and thorough information to
outside agencies 100% of the time when clients need different or
more appropriate care that HRMC cannot provide.
- Ensures therapy contacts are completed each month, according to
program standards.
- Provides diagnostic Psycho-Social Assessments
- Clinician completes intakes and assessments within 5 business
days of contact.
- Assessments will be completed in coordination with Assessment
Team and Client Benefits as evidenced by communicating any issues
concerning clients or assessment.Documentation requirements
- Ensures all family and client participation in treatment
planning as evidence by signatures on tx plans.
- Communicates to supervisor if referrals become low in weekly
supervisions.
- Meet weekly with direct supervisor to discuss clinical and
administrative concerns/ changes
- Create and implement treatment interventions that are reflected
in progress note documentation based on the treatment plan
goals
- Respond to all flags and e-mails within 2 business days
- Respond to all voice mails within 30 minutes
- Complete all required clinical documentation in accordance with
Center Policy and Procedure and funding source guidelines.
- Update treatment plans, crisis plans, DLA-20 and updated
admissions signature page every 6 months
- Complete and submit productivity summaries to supervisor by the
3rd business day of the month.
- Create daily progress notes and sign in EMR within one week of
service providedLeadership
- Attends and is timely to weekly multi-disciplinary team
meetings
- If licensed, signing the non-licensed clinicians' treatment
plans and being available for clinical consult
- Attends weekly staff meetings with team.
- Ensures update treatment plans, consumer strength assessments,
crisis plans, consent to contact, releases, ROI, MIRECC and updated
admissions signature page every 6 months as evidence by quarterly
quality record reviews for OnTrack TN clients
- Complies with plan to ensure tracking system for Tx plans for
all supervisees
- Complies with all required data for funding is tracked monthly
and quarterly, as requested by supervisor.
- Work with Team leader to develop and implement an ongoing plan
for community outreach to educate and connect others to the FEP
model of treatment.
- Attend trainings to advance skills in the FEP model, and ensure
cross-training of all appropriate roles, as part of the Coordinated
Specialty Care (CSC) model.This job description is not intended to

COMPENSATION:
- Starting salary for this position is approximately $24,100.50
/yr based on relevant experience and education.Equipment/Technical
Competency:
- Must have basic competency in office applications such as Word
and Excel.
- Position requires regular use of a computer, and appropriate
phone etiquette. Equipment/Technology:
- Position must be able to complete electronic documentation.
QUALIFICATIONS - .50 OnTrack Hamilton Master's ClinicianExperience
/ Knowledge:
- Master's degree in Human Services or related field. One-year
experience in the social work field or a behavioral health setting
preferred.
- Knowledge of etiology and treatment of mental illness, physical
and sexual abuse, suicide, human development, and cultural
diversity.Education / License:
- Master's degree in human services or related field with course
work or experience in the areas of cultural diversity, human
development, etiology and treatment of mental illness, alcohol and
drug abuse, physical and sexual abuse, suicide, and developmental
disabilities.Physical/Emotional/Social - Skills/Abilities:
- Exposure to biological hazards. Hearing of normal and soft
tones. Close eye work.
- Valid driver's license- F endorsement required for the
transportation of clients.
- Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. frequent
sitting, standing, walking, bending, stooping, and reaching.
- Required to be certified in, and effectively implement, verbal
and physical de-escalation techniques that include a wide range of
bodily movements; including but not limited to, grasping, holding
another person, going down on knees, running, and walking.Location:
